to the St. Joseph County United Way. Follow the instructions below. |

| For
tax purposes, make certain you print out 2 copies of the form,
using the second as your receipt. |

| 2.

|
| Choose
and fill out A., B., C., or D. on the pledge card. |
|

| 3.
|
|
| Send
1 copy to the United Way. |
|
| |
| 4.
|
|
| Print
and turn in an additional copy to your payroll office if you
select payroll deduction. |

| 5.
|
|
| You
may either fill out the Pledge Card online, then hit your browser's
print button, or hit your browser's print button, then fill
out the printed form. (Please fill out any requests for your
signature only AFTER you have printed the form). Proceed to
Pledge Card. After you have completed,
use your browser's back button to return to his page. |
